iCircle Services Profile and History

iCircle is a licensed non-profit Managed Care Organization offering health plans to individuals enrolled in Medicaid in 22 counties throughout the Western, Central, and Southern Tier regions of New York. Managed Care is a health insurance plan or healthcare system that coordinates the provision and ensures the quality of care and services for its members through a network of contracted, collaborat ive providers. iCircle Care is a Managed Long-Term Care (MLTC) plan. As a MLTC plan we work across multiple support systems to streamline the delivery of long-term care services for adults at least 18 years of age who are chronically ill or disabled; and who wish to stay in their homes and communities. Through these managed care plans iCircle will assure its members access to personalized, life-enriching hometown care for their healthcare and social support needs.
